About Our Service

Here at Transitions Care Fylde we understand how important it is for all adults to have a choice about how and where they live, and we appreciate the wish to remain at home for as long as possible. 
We also know the importance of knowing your loved one is cared for, safe and happy. With that in mind, our services are wholly centred on the wishes and needs of your family member. We offer a unique service due to our ethos of investing in the people who matter most; our team of Community Support Practitioners. We choose our staff very carefully and invest in training to enable them to offer the best possible service.

We offer a wide range of support which allows people to live in their own homes, giving you and your family member peace of mind. Every person has different requirements so we tailor our services to suit the individual. People being supported, and their families, are consulted at every stage while preparing their ideal care package and we offer flexibility so our service can alter as care needs change, sometimes at very short notice.

About Our Staff

Nothing is too much trouble. Our experienced staff ensure individual needs and preferences are looked after while maintaining dignity and privacy at all times. They work hard to promote well-being, independence and happiness. Our dedicated team treat everyone as though they are family and friends. They foster two-way relationships, getting to know each other and developing long lasting, caring bonds. No-one receives care without first being introduced to the member of staff who will be helping them, so there are no surprises, no unexpected visitors. 

Consistency is extremely important in helping your loved one feel settled and confident with their choices. We provide a Care Co-ordinator who is your main point of contact to ensure the service, and any changes, happen smoothly. They are the link between you and our company. Our team are very diverse and many have needed help themselves in one form or another, be that mental, emotional or physical support They truly understand the importance of the care they give. We are a close-knit team and celebrate and reward hard work whenever possible. We share reviews with our staff, so they see the difference they are making to people’s lives.

About Moving On

Transitions Care Fylde continues to care, when on occasion, people need to move on from our support. We have nurtured excellent relationships with the Care Quality Commission (CQC), local care homes and the local authorities. This means we can help you find the best nursing home should the need arise. Not only that, we help with the transition from home to full time care, so the move is less daunting.

All About Community

Here at Transitions Care Fylde we believe in supporting our local community with the service we offer, not only within our work ethos, but beyond that. We are ‘all about community’. We support many local projects and initiatives including Bay Trust Radio and Blackpool Palatine Rotary. Working with Local Authorities is key to providing quality care.

As an independent company Transitions Care are pleased to provide services for people living in Blackpool who are supported by Direct Payments and other funding streams. We have very good working relationships with the Blackpool Social Work teams.

Transitions are pleased to provide services for people who live in Lancashire who are supported by The County Council, we enjoy good working relationships with the Councils teams.

Blackpool & Fylde College

Training is key to delivering good quality services.

A number of our teams’ members have studied at Blackpool and The Fylde College on a range of courses from Introductory Care Programmes to Degrees in Health and Social Care. Transitions are involved in the College job fairs, and have employed team members directly from the College. Peter Brooks is a guest speaker at the College.

Bay Trust Radio – Graham Curry 70’s show

Transitions sponsor the 70’s show on Bay Trust Radio- an eclectic mix of songs from the era that brought us Disco, Punk and Prog. Check out and sing-along to the sounds that made the charts 40 years ago!

Sharing Knowledge, Experience and Research.

A number of the Transitions team are Alumni of Lancaster University, Peter Brooks is a supporter of the Defying Dementia team at the university and has presented lectures there.

Service Above Self.

The ethos of rotary is reflected in the ethos of Transitions, and as such, Transitions supports the Rotary club through attending events, fundraising, presenting and facilitating talks.

Fleetwood Town Flyers Walking Football Club

Onward Together.

The Fleetwood Town Flyers embodies and shares the same outlook as Transitions which is why we sponsor events at the club. “The Flyers” is a community organisation that sees the benefit of activity in the community and welcomes people of all abilities to take part in sports and social interaction for the wellbeing of it members.
